AFFORM.NET – AF Form 1075 – Casualty Personnel Roster – The Air Force Form 1075 is used to report the casualties of military personnel. It lists the qualified individuals within the Force Support Squadron who are qualified to report casualties and the 24-hour contact point for casualty reporting. The form is updated annually on 1 October. Where there is an active duty Casualty Accounting Representative (CAR), AF Form 1075 must also include the name of an alternate CAR who is capable of performing the casualty reporting mission when the primary CAR is not available.
