What is an Air Force Form 3559?
The AF IMT 3559 is a brief document that highlights a member’s entire career, highlighting significant performances and accomplishments. The form must be completed by the first general officer in the rating chain, whose ID is the Senior Rater ID. Past supervisors may also provide input, as long as the comments are commensurate with rank order in Section V. Other recommendations do not require comments.
