Your plan should also include a temporary custodian – someone who will take care of your child until your will is executed. This form is essential because it ensures your child goes to the caretakers you’ve chosen, and it’s important to include several back-ups in case of any unexpected issues.
AF Form 357 – Family Care Certification
AF Form 357 – Family Care certification is mandatory for all AF Technical School students, who are required to designate a short-term, long-term, and alternate caregiver for their child. This plan is also important in case of a member’s death, as a temporary custodian assumes custody of their child until the execution of that person’s will. By preparing a detailed Family Care Plan, students can ensure that their child will go to the care provider of their choice.
In addition to providing daycare to military children, the AF Aid Society program reimburses parents for child care at certified or affiliated AF family child care facilities. The program helps the entire base community by providing child care for families with active duty personnel. AF Form 357 – Family Care Certification is obtained from the Airman & Family Readiness Center (Bldg 400). The program is not meant to replace any other child care assistance.
