What is an Air Force Form 418 ?

In some cases, the Air Force will complete a military personnel flight (MPF) and a commander/civilian director fills out a corresponding form to deny continued service to a person. This type of form contains information that states the Airman was denied continued service and placed in RE code 2X, according to his or her Total Active Federal Military Service Date. It should not include diagnosis or follow-up care.

The Air Force Form 418 is used when an individual is reentering the service after being separated from service. For example, it’s used by individuals who separated from the military before Aug. 15, 1977, have an additional service requirement, and are not eligible to re-enlist in their current grade. In other cases, a person may be ineligible to re-enlist due to an alcohol abuse treatment program, failed to complete an orientation, or is otherwise ineligible to re-enroll for the reason listed.

Where Can I Find an AF Form 418 ?

If you are unable to locate an AF Form 418 in the Air Force’s computer system, you can fill out the form online. These forms are used by Airmen who are ineligible to remain in the Air Force. These documents are essential for submitting your application for continued service, and they are not only used for military purposes, but they are also a helpful resource for people who are ineligible to continue serving.

The AF Form 418 is the document that is used for requesting an honorable discharge from the Air Force Reserve. It is used to determine the status of a former airman whose eligibility is determined by his or her reentry code. The airmen are categorized according to their status – RE-2S and RE-2T – based on their tenure dates. This form also lists the reasons for an individual’s non-retention. These reasons can range from a derogatory judgment to the failure to perform duty duties.

AF Form 418 – Elective Reenlistment Program (SRP) Consideration/Denial Of Continued Service For Airmen

AF Form 418 is used to determine eligibility for CJR and reenlistment, and to reverse a non-selection decision. The commander must approve the reenlistment and certify the officer’s signature, date, and signature seal. The commander will make a final determination about the candidate’s eligibility for the SRP, and should consider the Airman’s performance, attitude, and willingness to adhere to the standards of service.

Applicants must meet the eligibility requirements for the CSRB program, including the age requirement, the high year of tenure, and counseling requirements. Once accepted, the reenlistment candidate must complete the CSRB. If the officer deems the Airman to be not eligible, they may appeal the decision. If the Airmen is denied a reenlistment based on this requirement, they can appeal the decision and be reenlisted within the 7-day period.
