What is an Air Force Form 4428?

This form is applicable to members of the Air Force, the Air National Guard, and the Reserve Command. It may also be required for retirees or those who have been separated from the Air Force. The Air Force has strict regulations regarding tattoos on members. Only certain body parts are permitted for tattoos. Ring tattoos are prohibited on the finger or hand unless they are under three-eighths of an inch in size. This means they are associated with extremist organizations or gangs that promote discrimination. If you have an obscene tattoo, you should notify your commander as soon as possible, as these are illegal. Additionally, unauthorized tattoos tend to bring discredit to the Air Force.

In order to obtain an AF Form 4428, you must submit a copy of the appropriate authorization. The AF Order Portal is unavailable July 12 – July 26, 2022.
